Graduation/Diploma Name Change

The University will allow graduating students to use chosen first names on University diplomas. Although the University considers diplomas to be ceremonial documents, in some instances external parties (i.e. employers, graduate programs, foreign countries, etc.) may only consider the diploma to be a legal document. Consequently, the chosen first name appearing on the diploma may conflict with other legal documents such as transcripts, driver licenses, etc., and cause issues that the graduating student is solely responsible to address. The University is not responsible for exposure of one’s legal name within its educational records or in managing the discrepancy between the graduating student and internal/external parties. Use this form to select a chosen first name for your Appalachian State University diploma if different than your legal first name. The name you request will also be the name announced and/or displayed at the Commencement Ceremonies.
