Forms for Withdrawal

What form do students fill out to withdraw? 

For a complete withdraw from this term, use the Withdraw From Current Term Form.

  • It is essential to speak with your advisor before withdrawing from all classes.
  • During the official withdrawal period, complete the "Withdraw from the Current Term Form" on the Registrar's website.
  • Requests are reviewed by the Student Success Team and then processed by the Registrar's Office once confirmed.
  • Readmission is required upon returning except when the student is dropping during a summer term(s).

If withdrawing before classes begin, use the Withdraw from Future Term Form.

  • Students who withdraw for both medical and non medical reasons should use the forms above.
